Please see attached important letter, you must complete an e-consent form for each child. If you have any questions about the vaccine please speak to the immunisation team on 0151 495 5066.
The school-aged immunisation teams are going to be using e-consent from September 2021. E consent allows parents and carers to complete consent forms on a computer or mobile phone. It will stop the need for school staff to distribute and collect consent packs / consent forms which can cause inconvenience and lead to information governance issues if / when forms are misplaced / lost.
